This Privacy Notice explains the types of personal data collected by Dyslexia Sense, how it is stored, handled and kept safe. It also details your rights, how to get in contact and how to complain in the event you have a problem or complaint. By providing your personal data, you accept and consent to the practices described throughout this notice.
Who I am
I am an independent specialist dyslexia teacher and Owner of Dyslexia Sense Limited. I offer diagnostic assessments, tuition and services for pupils, parents and schools.
What information is collected and used:
The information that is collected is needed to deliver the services you request, such as diagnostic assessment, tuition, or advice. This can include:

If you contact me as a parent/carer, information about your child

Background history of difficulties

Relevant medical conditions
How personal information is used
Your information is used solely to provide the services that you commission.
Who has access to your personal information
Your information is only shared with myself, and if requested/permission granted by the parent/carer, the child’s school. Your personal information will not be shared with third parties.
Lawful bases and data protection rights
Under UK data protection law, Dyslexia Sense Limited must have a “lawful basis” for collecting and using your personal information.
You have the right to request a copy of all or some of the information that is held about you. To ensure that your personal information is up to date and accurate you have the right to:

complain about the way your data is processed by us
If you would like to request information, correct information or complain about how your personal data is processed, this must be put in writing and sent to Rachel Lake at hello@dyslexiasense.com
How your personal information is kept secure
Information is stored on a password-protected computer or in secure storage.
How long information is kept for
Your personal data will be kept securely for 12 months after the final contact, at which point, it will be destroyed, except for your final dyslexia assessment report. This will be held for 7 years. During this time, an electronic copy of the report may be requested. After this time, it will not be possible for Dyslexia Sense Limited to supply further copies of the report.
