Terms & Conditions

Booking a Diagnostic assessment

  • Diagnostic assessments can be booked and confirmed by email.

  • A non-refundable deposit secures the agreed date. (Please see payment terms below).

Before the assessment

(Pre-16 years)

  • Once a date is secured and the deposit has been received, pre-assessment questionnaires will be sent via email to be completed by the individual’s parent/carer, the school and the individual themselves.

  • These must to filled in accurately and returned no later than the date 1 week prior to the assessment.

  • All information is kept completely confidential. Please note an assessment cannot go ahead unless this information is provided.

  • It is required that the individual being assessed should have had a standard eyesight test within the 12 months prior to the assessment date. Glasses must be brought to the assessment if prescribed.

During the assessment

  • All assessments take place on a 1:1 basis at the Dyslexia Sense premises.

  • The person being assessed is required to behave in a manner conducive to determining an accurate diagnosis. Where co-occurring needs mean that extra accommodations may be needed, this should be discussed prior to assessment.

  • Dyslexia Sense takes a child-centred approach to assessments - the comfort and needs of the individual being assessed will remain at the forefront of the process. Dyslexia Sense reserves the right to discontinue the assessment if the individual becomes distressed by

  • the process.

  • Parents/carers must be contactable for the duration of the assessment and must be available to collect their child early if necessary.

  • From time to time, further tests may be required following the assessment. If these are necessary, a time will be arranged following the assessment. Please note that no additional charge will be made in these circumstances.

After the assessment

  • The report will be completed up to 3 weeks following the assessment and will be supplied by email as a password protected PDF. This will be written to the required standard and adhere to the guidelines outlined by The SpLD Assessment Standards Committee (SASC).

  • If the person being assessed shows signs of a visual difficulty during the assessment, Dyslexia Sense reserves the right to withhold an assessment report until the extent/impact of any visual difficulties has been determined.

  • Minor amendments to the background information may be requested up to 14 days after a report has been received. Once a report has been finalised, the assessment process will be concluded.

Payment terms

  • A £200 deposit is paid on booking to secure the agreed assessment date, £200 is paid prior to the assessment taking place and the final balance (£175) is paid prior to the report being released.

  • Dyslexia Sense will be unable to release a report where full payment has not been made.

Cancellation Policy

Cancellation by the Client

  • The client may ask to rearrange the assessment, e.g. due to ill health, up to 24 hours before the appointment, at the earliest possible convenience to both parties.

  • Cancellation within 24 hours of a booked session will result in the loss of deposit.

Cancellation by Dyslexia Sense

  • Dyslexia Sense reserves the right to cancel a booked assessment for reasons of viability, adverse weather conditions, illness or any other unforeseen circumstances.

  • In the event that Dyslexia Sense has to cancel a session, clients will be given a minimum of 24 hours’ notice and arrangements will be made to reschedule the assessment.

Privacy Policy

  • Dyslexia Sense Limited adheres to Data Protection Guidance and is committed to protecting the privacy of individuals. Accordingly, all personal data collected will be subject to the Privacy Notice, available separately.

  • Following the assessment and the completion of the final assessment report, all client questionnaires, raw data test sheets and all other personal data will be permanently deleted/destroyed.

  • The final assessment report will be held by Dyslexia Sense for 7 years. During this time, an electronic copy of the report may be requested. After this time, it will not be possible for Dyslexia Sense to supply further copies of the report.
